Appointments & Office Hours
We are open from 9:00 am to 5:00 pm, Monday through Friday. If you are
unable to keep your appointment, please call 24 hours in advance.
After-Hour Phone Calls & Emergencies
If you need to contact the doctor after regular office hours or during our
lunch break, call our main office number, (310) 555-5555. Our answering
service will take a message or immediately forward all urgent messages to
Dr. Rahnama or the covering physician, whichever is appropriate to your
situation.
Medical Questions
Our staff has been trained to handle most routine phone calls. Should your
situation require additional professional information or advice, your request
will be forwarded to Dr. Rahnama, and he will return your call as soon as
possible or at a pre-arranged time.
Prescription Refills
Medication refills are handled only during regular office hours. Please call
your pharmacy for refills before you have depleted your supply, preferably
at least 48 hours in advance. In general, medicines are not prescribed over
the telephone without prior evaluation of the medical condition. If you are
overdue for an appointment, your prescription may not be refilled or only a
limited quantity may be authorized.
Medical Records
Your medical records are strictly confidential. We need your written
permission to release any information or to transfer your records.
Billing
We currently bill most insurance companies. We request a $100 first visit
deposit for patients without insurance coverage. During your first visit, you
can speak to our bookkeeper to arrange future payments. If your insurance
requires a co-payment, this should be paid at the time of your visit. If your
insurance company requires a written authorization or referral for you to see
a specialist, it is your responsibility to ensure that your primary care
physician has provided authorization prior to your visit with us.
